This form is to be filled out for any incident that occurs within Saskatoon Minor Softball League to include: CLICK HERE
- Ejections from any games or tournaments
- Includes of travesty of the game or incidents that are detrimental to the game of softball. IE: players, coaches, umpires or parents

ADOPT AN UMPIRE CLAIM SHEET
Adopt an umpire clarification,
Any teams using the adopt an umpire shall have them fill out the ticket at the bottom of the SMSL issued scoresheet, umpire to take the ticket and submit tickets along with adopt an umpire claim sheet form. Adopt an umpire claim sheet form can be found below. Please save these and submit at the end of each month to Todd Wosminity by taking a photo or scanning and emailing to smsltreas@gmail.com **Please keep all stubs for the month and submit once**
Any adopt an umpire that is registered as an umpire with Softball Saskatchewan will receive SMSL Umpire rates.

Emergency Action Plan
